La ineficiencia de la v\u00eda anaer\u00f3bica se compensa con el aumento del flujo de glucosa, un fen\u00f3meno observado por primera vez por Otto Warburg hace aproximadamente 80 a\u00f1os y actualmente explotado para la tomograf\u00eda por emisi\u00f3n de 2-fluoro-2-desoxi-D-glucosa-positr\u00f3n en radiolog\u00eda cl\u00ednica. Este \u00faltimo ha demostrado que el fenotipo glucol\u00edtico es un fen\u00f3meno casi universal en los c\u00e1nceres humanos.\u00a0<\/p>\n<p>El papel potencial del fenotipo glucol\u00edtico para facilitar la invasi\u00f3n tumoral se ha investigado a trav\u00e9s de modelos matem\u00e1ticos de la interfaz tumor-hu\u00e9sped. Los modelos de aut\u00f3matas celulares modificados y de reacci\u00f3n de difusi\u00f3n demuestran que los protones se difundir\u00e1n desde el tumor hacia el tejido normal peritumoral que somete a las c\u00e9lulas no transformadas adyacentes al borde del tumor a un pH extracelular significativamente m\u00e1s bajo de lo normal. Esto conduce a la muerte celular normal a trav\u00e9s de las v\u00edas de apoptosis dependientes de p53, as\u00ed como a la degradaci\u00f3n de la matriz intersticial, la p\u00e9rdida de las uniones intercelulares, la angiog\u00e9nesis mejorada y la inhibici\u00f3n de la respuesta inmune del hu\u00e9sped a los ant\u00edgenos tumorales.\u00a0<\/p>\n<p>Las c\u00e9lulas transformadas mantienen su capacidad proliferativa en pH extracelular \u00e1cido debido a mutaciones en p53 o alg\u00fan otro componente en las v\u00edas de apoptosis. Los modelos matem\u00e1ticos predicen nuevos enfoques de tratamiento centrados en la perturbaci\u00f3n del microambiente tumoral y est\u00e1n respaldados por datos cl\u00ednicos recientes que demuestran los beneficios de la azotemia y la acidosis metab\u00f3lica en la supervivencia de pacientes con c\u00e1ncer renal metast\u00e1sico.\u00a0<\/p>\n<p>La base evolutiva para la adopci\u00f3n del fenotipo glucol\u00edtico durante la carcinog\u00e9nesis sigue sin estar clara porque parece conferir desventajas competitivas significativas en las c\u00e9lulas tumorales debido a la producci\u00f3n de energ\u00eda ineficiente y al gasto de recursos para eliminar los subproductos \u00e1cidos.\u00a0<\/p>\n<p>Proponemos que el fenotipo glucol\u00edtico representa una adaptaci\u00f3n exitosa a los par\u00e1metros de selecci\u00f3n ambiental porque confiere la capacidad de invadir. Es decir, el fenotipo glucol\u00edtico permite que la c\u00e9lula se mueva del microambiente de una lesi\u00f3n premaligna al tejido normal adyacente. All\u00ed compite con las c\u00e9lulas normales que est\u00e1n menos en forma que las poblaciones dentro del tumor en un microambiente de abundancia relativa de sustrato.
