A pilot double blind randomized placebo controlled trial<\/a>. <i>Journal of Diabetes and its Complications<\/i>, 30(4), 723-727.<\/p>\n<p>ANTECEDENTES: La disfunci\u00f3n endotelial, la inflamaci\u00f3n vascular y el estr\u00e9s oxidativo se han relacionado integralmente con la patogenia tanto de la diabetes tipo 2 como de la enfermedad cardiovascular. En estudios anteriores se ha demostrado que el extracto de ajo envejecido (AGE), un potente antioxidante, aten\u00faa estos nuevos factores de riesgo en una poblaci\u00f3n no diab\u00e9tica.<\/p>\n<p>OBJETIVOS: Este estudio prob\u00f3 la hip\u00f3tesis de que los AGE pueden mejorar la funci\u00f3n endotelial, el estr\u00e9s oxidativo, la inflamaci\u00f3n vascular y la resistencia a la insulina en sujetos cardiovasculares de alto riesgo con diabetes tipo 2.<\/p>\n<p>METODOS: Se realiz\u00f3 un estudio piloto cruzado, doble ciego, controlado con placebo en 26 sujetos con diabetes tipo 2 que recibieron 1200 mg de AGE o placebo al d\u00eda durante 4 semanas con un per\u00edodo de lavado de 4 semanas. Se midi\u00f3 la HsCRP plasm\u00e1tica como marcador de inflamaci\u00f3n. Se midieron TAOS en plasma, GSH \/ GSSG en sangre y LHP en plasma como marcadores de estr\u00e9s oxidativo \/ defensa antioxidante. La resistencia a la insulina se midi\u00f3 mediante el m\u00e9todo HOMA-IR. La funci\u00f3n endotelial se midi\u00f3 mediante el cambio en el \u00edndice de reflexi\u00f3n (RI) post-salbutamol mediante fotopletismograf\u00eda digital y se midi\u00f3 la relaci\u00f3n alb\u00famina \/ creatinina en orina como un sustituto bioqu\u00edmico. Las mediciones se tomaron al inicio del estudio y despu\u00e9s de la intervenci\u00f3n con AGE o placebo.<\/p>\n<p>RESULTADOS: De los 26 pacientes estudiados (hombres 17, mujeres 9), la edad fue de 61 \u00b1 8 a\u00f1os (media \u00b1 1 DE), HbA1c 7,2 \u00b1 1,1%, PA 130\/75 \u00b1 15,9 \/ 9,8 mmHg, colesterol total 4,2 \u00b1 0,81 mmol \/ l, triglic\u00e9ridos 2,11 \u00b1 1,51 mmol \/ l y colesterol HDL 1,04 \u00b1 0,29 mmol \/ l. La mayor\u00eda de los pacientes estaban siendo tratados con metformina (59%), aspirina (50%) y estatinas (96%). El 36% fueron tratados con un IECA. No hubo cambios en estas terapias durante el estudio. El tratamiento con AGE no tuvo un efecto significativo sobre los par\u00e1metros metab\u00f3licos anteriores, incluida la resistencia a la insulina. El tratamiento con AGE tampoco tuvo un efecto significativo sobre los marcadores de funci\u00f3n endotelial (pletismograf\u00eda), estr\u00e9s oxidativo (TAOS, GSH \/ GSSG, LHP) o inflamaci\u00f3n (HsCRP).<\/p>\n<p>CONCLUSION: En este grupo de pacientes diab\u00e9ticos tipo 2 con alto riesgo cardiovascular, el tratamiento de 4 semanas con AGE no mejor\u00f3 significativamente la funci\u00f3n endotelial, la inflamaci\u00f3n vascular, el estr\u00e9s oxidativo o la resistencia a la insulina.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Atkin, M., Laight, D., &amp; Cummings, M.
